The Community Outreach Program has been designed to assist Tribal members with locating needed services. The coordinator will offer assistance with OHP applications, obtaining other resources and clinical enrollment assistance for all Cow Creek Health & Wellness Center CCH&WC eligible clients.

• Proactively encourage and assist patients that are potentially eligible for OHP benefits.
• Ensure application deadlines, information accuracy and completeness, and compliance of administrative rules.
• Assist all patients and Tribal members to the appropriate department when in need of assistance within the Tribal offices or with community outreach services that are available.
• Remain knowledgeable of community resources with assistance in food, shelter, clothing and safety.
• Act as Tribal Advocate to those in need of clarity in medical terminology assistance or customer service in all facets of programs needs.
• Encourage and educate on Tribal Events and activities.
